Who Can Get Private Health Insurance in Germany?
Not everyone can switch to PKV. The German system has clear rules about who qualifies. Here is what applies in 2026:
Employees earning over 73,800 EUR/year
This is the Versicherungspflichtgrenze for 2026. If your gross annual salary exceeds this threshold, you can opt for private insurance.
Self-employed and freelancers
No income threshold applies. You can choose PKV regardless of how much you earn.
Civil servants (Beamte)
The government covers 50 to 80 percent of costs through Beihilfe. PKV is the standard choice. More on civil servant health insurance.
Students
Can opt out of public insurance at the start of studies. See student PKV plans.

What Does Private Health Insurance Cost?
Your premium depends on your age, health status, and the coverage you choose. Younger, healthier applicants pay less.
Employees (around 30)
Starting from around 200 EUR/month. Your employer pays half, so you effectively pay around 100 EUR.
Self-employed
From around 300 to 600 EUR/month depending on age and coverage. No employer subsidy.
Civil servants
Often under 200 EUR/month thanks to Beihilfe covering the majority of costs.
Students
From around 80 EUR/month with student-specific plans.

How to Apply: Step by Step
Check your eligibility
Make sure you meet one of the criteria above. Employees need proof of income above the threshold. Self-employed need their business registration.
Compare plans
Use the comparison tool on this page. Enter your details and see plans from multiple insurers side by side. Look at coverage scope, deductibles, and premium stability, not just the monthly price.
Fill out the health questionnaire
Every PKV application includes a health questionnaire (Gesundheitsfragen). Answer honestly. Wrong answers can void your policy later. The insurer uses this to calculate your risk and premium.
Submit your application
Most insurers accept digital applications. You will need your ID or residence permit, proof of income or employment, and your completed health questionnaire.
Wait for approval
Standard processing takes 1 to 2 weeks. Some insurers with automated systems can approve within a few business days for applicants without pre-existing conditions.
Start your coverage
Once approved, coverage can start on the first of the following month. Cancel your old insurance (GKV) with the required notice period, usually 2 months to the end of a month.
Pre-existing Conditions: What to Expect
Having a health condition does not automatically disqualify you. But it changes the process:
- Minor conditions (allergies, corrected vision): usually no impact on approval or price
- Moderate conditions (controlled hypertension, past injuries): may result in a risk surcharge or exclusion for that specific condition
- Serious conditions: some insurers may decline, but others specialize in higher-risk applicants
The key is to apply to multiple insurers. Each one evaluates risk differently. For comprehensive coverage options, see our full PKV comparison.
