Health insurance is the first real adult decision most students face after moving to Germany. Not because it is the most interesting, but because nothing else moves forward without it. You cannot register at the university, finalise your Anmeldung, or apply for a residence permit without a valid insurance confirmation. So it gets done quickly, and usually without a proper comparison.

This page is the comparison you probably skipped. It covers both public insurance (GKV) and private insurance (PKV) for students in Germany in 2026, with the real 2026 rates, the rules on who can pick what, and the trade-offs that matter once the semester starts. If you want to jump straight to a personalised quote, use the comparison tool below; otherwise, keep reading.

Who this page is for

German and EU students, international students staying beyond 90 days, PhD candidates, dual-degree and second-degree students. If you are on a short-term visa, also read the international student section, the rules are different. Working expats above 30 may prefer the expat health insurance guide instead.

What students actually have to choose between

Every resident in Germany needs health insurance. No gap is allowed, not even for a week. For students, the choice narrows to three real paths.

1. GKV at the student rate

Available until the semester in which you turn 30, or for up to 14 semesters, whichever comes first. Most students use this.

2. Family insurance (Familienversicherung)

Free coverage through a parent's GKV policy up to age 25, as long as your earned income stays below 565 EUR per month in 2026 (603 EUR for Minijobs, updated in line with the 13.90 EUR minimum wage).

3. PKV with student tariff

You opt out of GKV within three months of enrolment. Once you opt out, you are locked in for the duration of your studies.

A fourth category that looks similar, but is not

Incoming insurance (Mawista, Care Concept, Dr-Walter) is valid for visa approval on short stays, but usually does not count as substitutive insurance once you are fully enrolled. See the international student section for the details.

What PKV typically adds for students

Faster specialist access

Private practices commonly prioritise PKV patients, which often shortens waiting times for specialist appointments.

Useful in cities with long waitlists

International coverage

Most student PKV plans include 6 to 12 months of worldwide cover per trip, suitable for Erasmus and internships in the EU.

Check country list per tariff

Age-based premiums

Below age 30, student tariffs stay in the 85-170 EUR/month range for healthy profiles, often below voluntary GKV once you age out of family insurance.

GKV student rate 2026: the statutory baseline

Every public fund charges the same basic student rate. What varies is the Zusatzbeitrag, the supplementary contribution each fund sets individually.

ComponentAmount (EUR/month)Basis
Base contribution87.3810.22% of reference income 854.67 EUR
Long-term care (childless)35.844.2% rate
Long-term care (with one child)30.033.52% rate
Zusatzbeitrag (average)~14.50~1.7% sector average, fund-specific
Typical total~123-137Before any fund bonus

The lowest student rates come from funds with a below-average Zusatzbeitrag (roughly 1.1-1.4%), BKK VBU, HKK and some smaller BKKs. The highest-charging funds sit around 137-140 EUR. The gap is small enough that most students pick based on service, English support or bonus programmes rather than on the Zusatzbeitrag alone.

Family insurance is still the cheapest option if you qualify: 0 EUR per month while your income stays below the limit and you are under 25.

Student PKV: the private alternative

If you are under 30 and starting your studies, you have a three-month window from enrolment to opt out of GKV in favour of PKV. After that window, you are committed to GKV at the student rate until you finish, you cannot change your mind mid-degree.

PKV student tariffs are priced by age, health, and the benefits you pick. Here is what a realistic 2026 picture looks like for a healthy student, based on published provider ranges.

The rule that costs people the most money

Switching from PKV back to GKV as an adult in Germany is restricted. As an employee, you can re-enter GKV only if your income falls below the JAEG (77,400 EUR per year in 2026) for a calendar year. Self-employed people cannot return through income alone. After age 55 a return is effectively impossible. As of January 2026, the Federal Ministry of Health closed several loopholes that PKV clients previously used.

The Deutsches Studierendenwerk puts it plainly: private insurance is a sensible alternative for a specific profile, but return to statutory insurance after graduation is only possible under certain conditions.

Translation for a 22-year-old picking insurance in first semester: choosing PKV now commits you to a PKV premium for the rest of your working life in most realistic career scenarios. Fine if your income trajectory is high and predictable. A problem if you end up on a regular salary later and cannot exit a 600-900 EUR/month adult tariff.

International students: what is different

If you are here on a visa, the insurance market you face is not the same one described above. There are three distinct situations.

EU/EEA students

Short stays: EHIC card is usually enough. Full semester: enrol in GKV or an accepted private plan. Swiss and some bilateral-agreement citizens have variations.

Non-EU, 90+ days

Most pick GKV at the student rate for simplicity. Classic German student PKV is an option if you meet the age and opt-out rules and plan to stay long term.

Language-school / short D-visa

Incoming insurance (Mawista, Care Concept, Dr-Walter, Expatrio packages) covers visa requirements. Not automatically substitutive PKV, expect to change insurance when you switch to a study visa.

Short correct answer: do not assume the insurance that worked for your visa is the right insurance once you are enrolled. Check with your university's student services before you pay for twelve months of anything. For broader English-language support and expat profiles over 30, compare expat health insurance in Germany or the English-speaking provider overview.

BAfoeG and the health insurance subsidy

If you receive BAfoeG and are not family-insured, the subsidy recognises insurance costs under §13a BAfoeG.

102 EUR
Statutory health insurance
35 EUR
Long-term care
137 EUR
Total monthly supplement

The supplement is a flat amount, not matched to your actual premium. If your GKV rate is 127 EUR and care contribution 36 EUR, the BAfoeG add-on of 137 EUR covers it in full. If you pick a PKV tariff at 180 EUR, the supplement pays 137 EUR and you cover the rest. Which is a useful reminder that "PKV is cheaper" depends heavily on what you are comparing against, including the BAfoeG allowance.

The older split of "94 EUR for under 30, 189 EUR for 30+" that some sites still list was the pre-2022 rate. It no longer applies. Source: §13a BAfoeG at bafoeg-rechner.de.

Study abroad, Erasmus and internships

GKV in the EU: emergencies covered via EHIC and some bilateral countries. Planned treatment and long stays usually are not. For Erasmus, most universities recommend a supplementary Auslandskrankenversicherung.

PKV worldwide: standard in most student tariffs, typically 6 to 12 months per trip. Longer stays or frequent travel may need a tariff with extended international protection.

Paid internships in Germany: above the Werkstudent earnings limit, normal GKV membership (employee rate) can kick in alongside or in place of the student rate. Check with the fund before signing the internship contract.

After graduation: your options

Once you deregister from the university, the student tariff ends. What happens next depends on what you do.

Employed below the JAEG

Mandatory GKV. If you were previously PKV, you have a one-time window to re-enter statutory insurance at the employee rate.

Employed above the JAEG

PKV (adult tariff) or voluntary GKV. Voluntary GKV is charged on your full salary up to the contribution ceiling.

Self-employed or freelance

No automatic GKV entry. Stay in PKV or join voluntary GKV, usually at a rate based on your actual earnings.

Civil servant (Beamte)

Beihilfe scheme applies and most Beamte take PKV with a Beamtentarif.

Unemployed with ALG I

Mandatory GKV membership at a subsidised rate.

PhD without employment contract

Stipendium: treated as student-equivalent. Assistant contract: you become an employee.

Anwartschaft: if you might move between Germany and another country, ask your PKV provider about Anwartschaft (dormancy). For a small monthly fee it preserves your rights and age-based premium while you are abroad, so you can reactivate without a new health check.

How to apply for student PKV, step by step

  1. 1

    Compare tariffs

Use the tool on this page or a provider form. Aim for 3-4 quotes to see the spread.

  2. 2

    Complete the health questionnaire

Answer honestly, undisclosed conditions can void the contract later.

  3. 3

    Receive the underwriting decision

Usually 3 to 7 working days for straightforward profiles.

  4. 4

    Sign the contract

You receive the Versicherungsbescheinigung confirmation letter.

  5. 5

    File the GKV opt-out

Antrag auf Befreiung von der Versicherungspflicht at a statutory fund within three months of enrolment. Mandatory even though you will not use the fund.

  6. 6

    Submit the PKV confirmation

Give it to your university student services or international office.

  7. 7

    Provide confirmation

If your residence permit requires it, hand the Bescheinigung to the Ausländerbehörde.

For GKV the process is simpler: pick a fund (TK, Barmer, DAK, AOK, a BKK), apply online, and the fund handles the confirmation to the university. Allow 1 to 2 weeks.

Trade-offs at a glance

Both options cover you. The difference is in the fine print.

For the full GKV vs PKV overview across all life stages, see our health insurance comparison guide.

Specialist access

PKV patients often see specialists sooner, with the gap most noticeable in rural Bavaria and smaller cities.

Dental and vision

Student PKV typically reimburses 80-100% of basic dental and provides 150-300 EUR/year for glasses. GKV covers far less here.

Travel and abroad

PKV includes worldwide cover (time-limited). GKV via EHIC covers EU emergencies; Erasmus usually needs a supplement.

Frequently asked questions

Based on real queries from students searching in 2026.

1
How much does health insurance cost for students in Germany in 2026?

Statutory insurance (GKV) at the student rate runs about 123-137 EUR per month including Zusatzbeitrag and long-term care. Private student tariffs (PKV) are 95-140 EUR at ages 20-24 and 110-170 EUR at ages 25-29 for healthy profiles. Family insurance is free up to age 25 as long as your earned income stays under roughly 535 EUR/month.

2
What is the income threshold for switching to private insurance in 2026?

The JAEG (Jahresarbeitsentgeltgrenze) is 77,400 EUR per year or 6,450 EUR per month for 2026, up from 73,800 EUR in 2025. As an employee you need to earn above this figure for a full calendar year before you qualify to switch. As a student, the threshold does not apply, you can opt into PKV separately within three months of enrolment.

3
How does the BAfoeG health insurance subsidy work in 2026?

Under §13a BAfoeG, you receive 102 EUR per month toward statutory health insurance plus 35 EUR for long-term care, a total of 137 EUR per month. The amount is flat and independent of your actual premium. If you are still family-insured through a parent, you do not receive the supplement.

4
Am I required to have health insurance as an international student in Germany?

Yes. Every resident, including international students, needs substitutive health insurance from day one. EU students can rely on EHIC for short stays but must register into GKV or an accepted private plan for full semesters. Non-EU students usually pick GKV or a qualifying PKV; for short-term language-school visas, incoming insurance products are accepted but are not the same as substitutive German PKV.

5
Can I switch back to GKV if I pick private insurance as a student?

Only under restricted conditions. After graduation, you can return to GKV if you become an employee with a gross salary below the JAEG (77,400 EUR in 2026). Self-employed or higher-salary paths keep you in PKV. After age 55, a return is effectively impossible. The rules tightened again in January 2026, so treat a student PKV decision as close to permanent.

6
What happens to my private insurance after graduation?

When you deregister from the university, your premium moves from the student rate to an adult rate, which is higher. If you then take an employed job below the JAEG, you re-enter GKV. If you earn above it or go self-employed, you stay in PKV on an adult tariff. Civil servants (Beamte) keep PKV under the Beihilfe scheme.

7
Does private student insurance cover study abroad?

Most student PKV plans include worldwide coverage for 6 to 12 months per trip, with emergencies covered throughout. Longer stays typically need a rider or a dedicated international plan. GKV covers EU emergencies via EHIC; for longer non-EU programmes you usually need supplementary insurance.

8
Is private insurance worthwhile for PhD students?

Often, yes. By PhD age you have usually aged out of family insurance and possibly out of the 14-semester GKV student rate. Voluntary GKV at that point is based on imputed income and can be 200+ EUR per month. A comprehensive PKV tariff for a healthy 28 to 32 year old competes with that rate and offers faster specialist access. It is still a long-term decision.

9
What does PKV cost for a 30-year-old student?

For a healthy 30 year old in a comprehensive student tariff, expect 160-230 EUR per month in 2026. Above 34, providers typically move you to an adult private tariff starting around 230-330 EUR for comparable cover. Pre-existing conditions, chosen deductibles and optional modules move the premium up or down.

10
Can I insure my spouse and children on my private policy?

Private insurance has no free family coverage, each member needs a separate contract. Partner policies start around 180 EUR per month; children from 80-120 EUR depending on tariff. If you plan to start a family soon, the free family insurance in GKV is a meaningful financial difference.

11
Which insurer is best for international students in Germany?

It depends on visa status and length of stay. For long-term EU and non-EU students who want English service, Allianz and Ottonova are common picks. For language-school and other short visas, incoming insurance from Mawista, Care Concept or Dr-Walter satisfies the visa requirement but is not substitutive German PKV. Check with your university's international office and the DAAD overview before signing.

12
Where can I get English-language support for German health insurance?

Ottonova operates fully in English. Allianz, AXA, DKV, HanseMerkur and Signal Iduna have English-speaking teams. Among statutory insurers, TK, Barmer and DAK publish English-language material and handle expat questions in English, though daily admin is mostly in German.
