What students actually have to choose between
Every resident in Germany needs health insurance. No gap is allowed, not even for a week. For students, the choice narrows to three real paths.
1. GKV at the student rate
Available until the semester in which you turn 30, or for up to 14 semesters, whichever comes first. Most students use this.
2. Family insurance (Familienversicherung)
Free coverage through a parent's GKV policy up to age 25, as long as your earned income stays below 565 EUR per month in 2026 (603 EUR for Minijobs, updated in line with the 13.90 EUR minimum wage).
3. PKV with student tariff
You opt out of GKV within three months of enrolment. Once you opt out, you are locked in for the duration of your studies.

The lowest student rates come from funds with a below-average Zusatzbeitrag (roughly 1.1-1.4%), BKK VBU, HKK and some smaller BKKs. The highest-charging funds sit around 137-140 EUR. The gap is small enough that most students pick based on service, English support or bonus programmes rather than on the Zusatzbeitrag alone.
Family insurance is still the cheapest option if you qualify: 0 EUR per month while your income stays below the limit and you are under 25.

The rule that costs people the most money
Switching from PKV back to GKV as an adult in Germany is restricted. As an employee, you can re-enter GKV only if your income falls below the JAEG (77,400 EUR per year in 2026) for a calendar year. Self-employed people cannot return through income alone. After age 55 a return is effectively impossible. As of January 2026, the Federal Ministry of Health closed several loopholes that PKV clients previously used.
The Deutsches Studierendenwerk puts it plainly: private insurance is a sensible alternative for a specific profile, but return to statutory insurance after graduation is only possible under certain conditions.
Translation for a 22-year-old picking insurance in first semester: choosing PKV now commits you to a PKV premium for the rest of your working life in most realistic career scenarios. Fine if your income trajectory is high and predictable. A problem if you end up on a regular salary later and cannot exit a 600-900 EUR/month adult tariff.
International students: what is different
If you are here on a visa, the insurance market you face is not the same one described above. There are three distinct situations.
EU/EEA students
Short stays: EHIC card is usually enough. Full semester: enrol in GKV or an accepted private plan. Swiss and some bilateral-agreement citizens have variations.
Non-EU, 90+ days
Most pick GKV at the student rate for simplicity. Classic German student PKV is an option if you meet the age and opt-out rules and plan to stay long term.
Language-school / short D-visa
Incoming insurance (Mawista, Care Concept, Dr-Walter, Expatrio packages) covers visa requirements. Not automatically substitutive PKV, expect to change insurance when you switch to a study visa.
Short correct answer: do not assume the insurance that worked for your visa is the right insurance once you are enrolled. Check with your university's student services before you pay for twelve months of anything. BAfoeG and the health insurance subsidy
If you receive BAfoeG and are not family-insured, the subsidy recognises insurance costs under §13a BAfoeG.
The supplement is a flat amount, not matched to your actual premium. If your GKV rate is 127 EUR and care contribution 36 EUR, the BAfoeG add-on of 137 EUR covers it in full. If you pick a PKV tariff at 180 EUR, the supplement pays 137 EUR and you cover the rest. Which is a useful reminder that "PKV is cheaper" depends heavily on what you are comparing against, including the BAfoeG allowance.

Study abroad, Erasmus and internships
GKV in the EU: emergencies covered via EHIC and some bilateral countries. Planned treatment and long stays usually are not. For Erasmus, most universities recommend a supplementary Auslandskrankenversicherung.
PKV worldwide: standard in most student tariffs, typically 6 to 12 months per trip. Longer stays or frequent travel may need a tariff with extended international protection.
Paid internships in Germany: above the Werkstudent earnings limit, normal GKV membership (employee rate) can kick in alongside or in place of the student rate. Check with the fund before signing the internship contract.
After graduation: your options
Once you deregister from the university, the student tariff ends. What happens next depends on what you do.
Employed below the JAEG
Mandatory GKV. If you were previously PKV, you have a one-time window to re-enter statutory insurance at the employee rate.
Employed above the JAEG
PKV (adult tariff) or voluntary GKV. Voluntary GKV is charged on your full salary up to the contribution ceiling.
Self-employed or freelance
No automatic GKV entry. Stay in PKV or join voluntary GKV, usually at a rate based on your actual earnings.
Civil servant (Beamte)
Beihilfe scheme applies and most Beamte take PKV with a Beamtentarif.
